All new clients have the ability to create an account with our client portal to access forms digitally. Clients will be invited to access the client portal by their therapist via email, or clients can access the portal here. After creating an account in the portal, please download the new client forms, complete the forms with information and signatures, and upload into the portal.

Note: Forms must be completed 24-hours prior to the initial session, or the initial session will be cancelled until completed forms are submitted.
